SOCIAL PRESCRIBING LINK WORKER

We currently have an exciting opportunity for a self-motivated person
to work with us in the role of SOCIAL PRESCRIBING LINK WORKER.
Wexford Local Development have been contracted by the HSE to be the
host organisation for the employment of the Social Prescribing Link
Worker as part of the Sl?intecare Healthy Communities Initiative.
This initiative is focussed on the implementation of an enhanced
health and wellbeing programme within areas that have the greatest
levels of disadvantage and highest proportion of young families to
deliver evidence informed services to improve local population health
and wellbeing outcomes. In Co. Wexford the programme will initially
focus on four areas in Enniscorthy Urban and Enniscorthy Rural
Edermine and Wexford Rural Taghmon and Wexford Urban.
The post holder will be responsible for working with individuals on a
one to one basis to improve health and wellbeing as well as working in
partnership with health professionals and the community and voluntary
sector. Social Prescribing involves a referral from a healthcare
professional and an agreed referral to a local community activity or
service following a consultation with the referred person.
The position is full time and is being offered on a fixed term

experience and skills.
APPLICANTS FOR THE POSITION WILL REQUIRE THE FOLLOWING;
* A THIRD LEVEL QUALIFICATION IN SOCIAL, COMMUNITY, HEALTH OR RELATED
FIELD.
* HAVE A MINIMUM OF 3 YEARS EXPERIENCE IN A COMMUNITY DEVELOPMENT,
HEALTHCARE OR RELATED FIELD.
* DIRECT EXPERIENCE OF SUPPORTING PEOPLE ON A 1:1 OR IN A GROUP
CAPACITY
* HAVE EXPERIENCE BUILDING RELATIONSHIPS ACROSS A VARIETY OF
ORGANISATIONS
* THEY WILL HAVE A KNOWLEDGE OF REDUCING HEALTH INEQUALITIES AND AN
UNDERSTANDING OF THE WIDER DETERMINANTS OF HEALTH.
* HAVE EXCELLENT COMMUNICATION SKILLS, ANALYTICAL AND PROBLEM
SOLVING SKILLS.
* PROFICIENT ADMINISTRATION, ORGANISATIONAL AND COMPUTER LITERACY
SKILLS.
* ACCESS TO THEIR OWN TRANSPORT AND A FULL CLEAN DRIVING LICENCE AS
TRAVEL WILL BE ESSENTIAL TO THE ROLE.
